Requirements
Must be completed before participating in any team activities
- Register as a volunteer member of USA Hockey.
- Complete the USA Hockey Safe Sport Training. Required every year.
- Complete the USA Hockey Background Screening. Required every 2 years.
- Complete the City of Rocky River Authorization to Release Information form. Please complete the form linked below and email a scanned copy to Daniel Conway for processing.
